Required Documents Checklist
To help your child begin their journey with Ascend ABA Support, please gather the following items for our intake team:
Formal Diagnosis Report
A full diagnostic evaluation report (usually from a Psychologist, Neurologist, or Developmental Pediatrician) confirming a diagnosis of Autism Spectrum Disorder (ASD).
Referral or Prescription
A written order or “Script” for ABA Therapy from your child’s primary doctor or diagnosing specialist.
Insurance Card
A clear copy of the front and back of your current primary (and secondary, if applicable) insurance card.
Individualized Education Program (IEP)
If your child receives services through their school, a copy of their current IEP helps us ensure our goals align with their educational support.
Previous Clinical Reports:
Any recent Speech (SLP), Occupational (OT), or Physical Therapy (PT) evaluations that help us see the “full picture” of your child’s development.
Photo ID
A copy of a parent or guardian’s government-issued identification.
